Los protocolos de recuperacion intensificada (PRI) en cirugia de colon y recto (QCR) disminuyen la estancia hospitalaria (EH) y las complicaciones, a traves de la reduccion del estres perioperatorio y la mejora de la practica clinica. El PRI en QCR electiva en el Hospital Universitario de Guadalajara (HUG) fue implantado en Abril 2016, e incluye 22 variables de las incluidas en la guia RICA publicada por el Ministerio de Sanidad, Servicios Sociales e Igualdad. Se establecieron dos grupos: uno previo al PRI (grupo preRICA: 135 pacientes operados de cirugia electiva de colon y recto del 1 de Enero 2015 al 31 de Diciembre 2015 segun la practica clinica habitual; y otro tras la implantacion del PRI (grupo RICA: 121 pacientes operados de QCR electiva del 1 de Mayo del 2016 al 31 de Enero 2017. El grado de satisfaccion de los pacientes se analizo mediante la encuesta de la guia RICA. Para valorar el efecto del PRI en la respuesta inflamatoria sistemica a la cirugia (RIS), se utilizo la proteina C reactiva (PCR) como marcador objetivo, y se comparo su valor en ambos grupos. Se puede concluir que la aplicacion de un PRI en cirugia electiva de colon y recto es seguro y efectivo en el HUG.
